Prior to beginning Awaken180 you need to fill out a health profile that gives us some information regarding any medical conditions you may have as well as some general background information to make your initial consultation as smooth as possible.
Filling Out a New Health Profile:
- Head over the new health profile page
- You will see a basic form intended to collect some general background information (all required fields are marked with a "*").
- Click Continue to head to the next section.
Note: You will receive an email with a link to the next section after clicking "continue" in case you need to get back to your health profile.
- After clicking "continue" you will be redirected to the next page which loads the extended portion of the health profile which is mandatory and should take about 10 minutes to fill out. We recommend filling out the form from a computer as the form is large and can cause issues on phones with slow internet connections.
Note: Please complete the form truthfully and in as much detail as possible so that the information is accurate.
- Please refresh the page if you do not see the following image within 10 seconds.
- Complete the extended portion of the health profile until you see the page with the text: "You did it! Thank you for taking the time to complete your Health History Profile. You can now leave the page without losing any data!"
- That's it! A member of our team will be in touch to schedule a consultation if one has not already been scheduled!
- If you exited your health profile and lost your place, look for an email from us in your email inbox. If you don't see anything in your inbox, check your spam. You will see an email that has a link to resume your health profile.
- If you don't see this email, please speak to a member of our team! We'll be able to resend it for you
